Revision rhinoplasty with rib cartilage, diced cartilage fascia (DCF) and silicone implant removal
2-Stage Revision Rhinoplasty This patient had a silicone implant that was thinning the skin and nearly protruding. In the first stage, we removed the implant to allow the skin to heal and strengthen. Once stable, we performed a revision rhinoplasty using the patient’s own rib cartilage and DCF (diced cartilage fascia) to reconstruct the nose and restore natural contour.

Male revision rhinoplasty with rib cartilage, diced cartilage fascia (DCF) and alarplasty
This male patient presented after a prior rhinoplasty with significant structural weakness, including a thin nasal bridge, inadequate support, and poor projection. The lack of stable cartilage framework resulted in a nose that appeared under-projected and structurally compromised, particularly for a male patient where strength and definition are critical. He underwent male revision rhinoplasty with alarplasty performed by Dr. Donald Yoo, utilizing DCF (diced cartilage fascia) rib cartilage to fully rebuild the nasal framework. Rib cartilage was selected due to its strength and reliability in revision cases where native cartilage is insufficient. The DCF technique allowed for precise reconstruction of the nasal bridge, restoration of projection, and smooth contouring while maintaining long-term structural stability. Alarplasty was performed concurrently to address nostril width and improve overall nasal balance. The revision focused on rebuilding support from the foundation up, restoring projection, and achieving a natural yet masculine result with durable structural integrity. -

Revision rhinoplasty with rib cartilage and diced cartilage fascia (DCF)
This patient presented after a prior rhinoplasty that resulted in significant thinning of the nasal skin, loss of structural support, and an unnatural nasal contour. The weakened framework led to visible irregularities and collapse, creating an appearance that lacked both stability and balance. The patient underwent revision rhinoplasty performed by Dr. Donald Yoo, utilizing rib cartilage with the DCF (diced cartilage fascia) technique to reconstruct the nasal framework. Rib cartilage was selected due to its strength and reliability in revision cases where native cartilage has been compromised or depleted. The DCF technique allowed for smooth contouring while restoring volume and structural integrity beneath thin skin. The revision focused on rebuilding support, correcting contour irregularities, and restoring a more natural nasal appearance, with careful attention to long-term durability and skin tolerance. This approach provides a stable foundation while minimizing visibility through thin skin, resulting in a balanced and natural-looking outcome. -

Before and After revision rhinoplasty with rib cartilage and DCF + silicone implant removal
This patient presented after a prior silicone implant rhinoplasty that resulted in irregular nasal contour, visible implant edges, and retraction, creating an unnatural appearance and imbalance with the rest of her facial features. Over time, the implant contributed to contour distortion and soft-tissue compromise, which is a known risk in implant-based rhinoplasty, particularly in thinner skin patients. She underwent revision rhinoplasty performed by Dr. Donald Yoo, with complete removal of the silicone implant and reconstruction of the nasal framework using rib cartilage and the DCF (diced cartilage fascia) technique. Rib cartilage was chosen for its strength, biocompatibility, and reliability in complex revision cases, allowing for full structural rebuilding rather than replacement of another implant. The DCF technique was used to restore smooth dorsal contour, address retraction, and provide a stable, natural-appearing bridge under compromised soft tissue. In addition to nasal reconstruction, facial balancing principles were applied to ensure the revised nose harmonized with the patient’s overall facial proportions, resulting in a more natural and cohesive appearance. The revision focused on eliminating implant-related irregularities, rebuilding stable support, correcting retraction, and achieving balanced, long-term results using the patient’s own tissue rather than synthetic material. -

Revision rhinoplasty with rib cartilage
After two previous rhinoplasties this patient was left with alar retraction, pinched nostrils, and significant nasal obstruction. Restoration revision rhinoplasty with rib cartilage and composite grafts from the ears were used to improve the airway and restore a natural appearance to his nose.

Revision Rhinoplasty (revision nose job) is for patients who have already had rhinoplasty and want to improve how their nose looks or works. Board-certified, fellowship-trained facial plastic surgeon Dr. Donald B. Yoo specializes in these more complex cases – carefully rebuilding support, addressing scar tissue or prior implants, and shaping a more natural, lasting result with your own tissue when possible, such as rib cartilage. Whether you’re hoping to restore balance after an unsatisfactory outcome or feel more comfortable in your own skin again, he plans each revision around your anatomy and goals, with clear guidance as you heal.
