The goal is a nose that fits your face and keeps features that reflect your heritage.

Silicone implants were the most common approach from the 1970s through the early 2000s. They are simple and quick, but carry higher risks of infection, shifting, visibility and extrusion (the implant pushing through the skin).

Many surgeons now favor your own cartilage. In Dr. Donald Yoo’s practice, a common choice is diced cartilage fascia (DCF), made from your own rib cartilage and a thin layer of temporalis fascia, which is the tissue over the temple muscle.

Planning matters as much as technique: computer morphing helps you and your surgeon agree on the goal, but it is a communication tool, not a promise.

The most common problem after dorsal augmentation is too much or too little height, which is why a detailed discussion before surgery is so important.

Recovery in Dr. Yoo’s practice: about 1 week with a cast and stitches, about 2 weeks before most people feel comfortable returning to work or school, and 3 to 6 months for most of the swelling to resolve.

Read the Asian rhinoplasty service page for an overview of the procedure, or see results in the Asian rhinoplasty before and after gallery.

What makes Asian rhinoplasty different?

Every nose is individual, but Dr. Yoo describes several features that are common in Asian patients: a smaller nasal pyramid with shorter nasal bones, a deeper nasofrontal angle and a lower nasal starting point (where the bridge begins between the eyes), a thinner and weaker septal cartilage (the cartilage wall inside the nose), less tip projection, and a short nose. The dorsal aesthetic lines, the soft lines that run from the brow down each side of the bridge to the tip, may be wide and indistinct. That is why raising the bridge is such a frequent and important part of surgery.

Beauty goals matter too. Patients are not looking to erase their ethnicity. The aim is to blend features you value from both traditions into a nose that looks natural on your face.

Read more: Contemporary Approaches in Asian Rhinoplasty: What’s Changed, What’s the biggest challenge in Asian rhinoplasty? and Are there differences between male and female Asian rhinoplasty?. Dr. Yoo also co-authored a 2024 review of Asian male blepharoplasty and rhinoplasty (Frederick, Kim and Yoo, 2024). Watch a male Asian rhinoplasty before and after.

From silicone to your own cartilage: a short history

Surgeons in Asia experimented with many materials to raise the bridge, including paraffin, ivory and jade. In 1964, Dr. Boo-Chai Khoo described a technique using silicone implants, and silicone became the most common method through the 1970s and into the early 2000s. These implants gave a quick increase in height, but often a nose that looked unnatural.

Over time, complications such as infection, shifting, thinning of the skin and extrusion led many surgeons to move toward your own tissue. Cartilage from the ear, septum or rib became popular, along with an important tip technique called the septal extension graft, which controls how far the tip projects.

Silicone and other implants

A single silicone implant remains in use because it is simple and saves time and cost. But the same implant carries a higher risk of infection, migration, visibility and extrusion than your own tissue, and over time the skin over it can thin and show the edge of the implant. Late changes such as calcium deposits on the implant can also cause irregular contours. Patients choosing this route should know that implants are not permanent and that more surgery should be expected. One example in Dr. Yoo’s chapter is a silicone implant removed after 40 years.

Some surgeons combine an implant on the bridge with your own cartilage at the tip, or wrap the implant in fascia or dermis to reduce its movement. These hybrid methods lower the risk compared with a single implant, but synthetic implants still have higher rates of infection, migration, extrusion and visibility than grafts made of your own tissue.

Rib cartilage and diced cartilage fascia

Rib cartilage is a generous source of strong, straight cartilage, which is often needed for both the bridge and the tip. Placed as one solid piece, it can warp, so careful carving and selection of the part of the rib that resists bending are important.

Diced cartilage fascia (DCF) was developed to avoid warping. The cartilage is cut into very fine particles (less than half a millimeter), placed in a sheath made from temporalis fascia and shaped to the nose. It has become a widely accepted method for raising the bridge in first-time Asian rhinoplasty because of its aesthetic results, low rate of complications and permanence, as described in Dr. Yoo’s chapter. He has also published an updated version of the technique (Wu and Yoo, 2019).

How it works in your surgery, in plain terms:

Planning. You and Dr. Yoo agree on the starting point of the bridge, the profile and the width of the dorsal aesthetic lines, often using computer morphing.

Cartilage. Septal and ear cartilage can supply mild to moderate augmentation. Rib cartilage is generally used when moderate or significant augmentation is needed. The rib is reached through an incision of about 1 cm in the crease under the breast (the inframammary fold).

Fascia. The temporalis fascia is taken through an incision hidden in the hair.

Shaping. The wrapped graft is shaped with fine stitches to match your planned dorsal aesthetic lines, then molded with a heat-softened splint during surgery.

Because DCF needs both cartilage and fascia, it involves two donor sites, and Dr. Yoo notes this is its main disadvantage compared with other grafts.

<iframe loading="lazy" title="Asian Rhinoplasty with Dr. Donald B. It helps you and your surgeon talk about how much height you want, where the bridge should start, whether the profile should be straight or gently sloped and how wide the dorsal lines should be. It also helps show how the nose relates to your forehead, eyebrows, lips and chin.

Recovery and what to expect

Swelling after DCF lasts longer and can be greater than after a solid graft. Swelling can hide small problems in the first 1 to 2 years, so the final shape is judged slowly. During the first 2 months, Dr. Yoo may use injections such as Kenalog (a steroid) and 5-fluorouracil, along with digital massage, to guide the swelling down. In Dr. Yoo’s practice, patients wear a cast and have stitches for about 1 week, feel comfortable going back to work or school after about 2 weeks, and see most of the swelling resolve over 3 to 6 months. Watch Asian Rhinoplasty, Nose Job Recovery, Rib Cartilage + DCF.

Risks and complications

Dr. Yoo’s chapter groups complications into three stages:

Before surgery: a mismatch between what the patient imagines and what the surgeon plans. Words like “natural” or “subtle” mean different things to different people, so standardized photos and computer morphing help.

During surgery: rib cartilage can occasionally be too calcified to use. In Dr. Yoo’s experience this affects about 1 in 200 Asian patients, more often with older age, prior radiation or trauma, or long-term waist trainer wear. Careful sizing, shaping and placement of the graft also matter.

After surgery: swelling can mask contour irregularities early on. Minor irregularities can be treated after healing, and the most frequent problem is over- or under-augmentation. Over-augmentation may need the graft reduced or removed, and under-augmentation may need more cartilage.

Changing the nostrils

A wide or flared nostril base can be reduced in the same operation, which can balance the bridge and tip work.
