Both operations tighten the deeper layer so the lift does not rely on pulling the skin alone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In a SMAS facelift, the surgeon tightens, folds or repositions that layer. In a deep plane facelift, the surgeon releases the layer together with the skin and the supporting ligaments below it and moves it as one unit, which can lift the cheeks and jowls from a deeper level. Their findings do not all line up, which is itself useful for patients to know.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Patient satisfaction and complications.<\/strong> A review of 21 studies and 2,896 patients found patient-rated satisfaction of 94.4% after deep plane facelift and 87.8% after SMAS facelift. The overall complication rate was 17.2% for deep plane and 10.3% for SMAS. The authors concluded that both techniques give robust, long-term results with high satisfaction (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40801931\/\">Khoury and colleagues, 2025<\/a>).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Safety.<\/strong> A review of 47 studies and 10,766 patients found hematoma (a collection of blood under the skin) in 3% of deep plane and 2% of SMAS facelifts. Infection was low with both. Most nerve injuries were temporary, and permanent nerve injury was rare. Only one study compared the two techniques directly, and it found better midface rejuvenation with the deep plane. The authors concluded that safety was comparable and that the choice should be individualized (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40600822\/\">Vayalapra and colleagues, 2025<\/a>).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Deep plane outcomes.<\/strong> A review of 45 studies and 10,784 patients who had deep plane facelifts found an overall complication rate of 6.6%. No permanent facial nerve injury was reported. Temporary facial nerve injury occurred in about 1.2% of patients, hematoma in 1.8%, infection in 0.8% and skin necrosis in 0.1%. Aesthetic improvement was greater with deep plane than with SMAS or skin-only techniques, although the way results were judged was subjective. The authors called for randomized studies and long-term follow-up (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/42576065\/\">Koroma and colleagues, 2026<\/a>).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How to read these numbers.<\/strong> The complication rates range from 6.6% to 17.2% for deep plane, probably because the studies defined and counted complications differently. Your anatomy, how much laxity you have in the cheeks, jowls and neck, your skin quality, your goals and your health all matter. In Dr. Yoo\u2019s experience, most patients feel comfortable in public with their hair styled about 2 weeks after surgery, and comfortable wearing their hair up, with the incisions near the ear and hairline visible, after about 4 to 6 weeks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A well-performed facelift usually lasts about 10 to 15 years. It does not stop aging, so your face continues to change naturally afterward, but it starts from a younger place.
